To sign up all you need to do is download these forms from Charities Aid Foundation, a payroll giving agency, fill them in, and send them as directed.

Employers

Contract Form - please print and complete twice. One will be for you, the other for CAF. Completion guidlines and help

Payroll Form - ask your payroll personnel to complete and return to CAF with the contracts. Completion guidlines and help

Grant Form - to cover your administration costs. See here for elegibility.

Optional other forms - if you wish to match staff donations or pay the administration costs. Please note, some of these options entail a fee, for further information click here

Alternatively if you would like CAF to send you an introduction pack of all the above documents then please fill in your details.

What happens next?

Once completed and returned to CAF these forms will register your participation in the scheme with the inland revenue. Once all the forms are processed and the first deductions are made you will recieve your grant.
When everything is set up you can then contact ACT and ask us to help get employees signed up. This could be anything from promotional materials to one-to-one and small group staff information days.

Employees

First make sure your employer is registered with a payroll giving agency such as CAF. Once registered just return the following form to your payroll department.
